મહત્વની સૂચના
1. ક્રિકેટનું ફોર્મ ભરનાર દરેક જણે પોતાનો લેટેસ્ટ પાસપોર્ટ સાઈઝનો ફોટો અપલોડ કરવો
2. પેમેન્ટ નો સ્ક્રીનશોટ ફોર્મમાં એટેચ કરવાનો છે જે વંચાય તેવો હોવો જોઈએ અને તેમાં ટ્રાન્સેકશન ID દેખાય તેવો હોવો જોઈએ
3. કોઈ પણ ડમી અટેચમેન્ટ ચાલશે નહિ
ઉપરની બંને બાબતો ખાસ ધ્યાન માં રાખવી, જો બંનેમાંથી એક પણ વસ્તુ બરાબર નહિ હોય તો આપનું ફોર્મ રિજેક્ટ થઇ શકે છે અને તમને કોઈ રિફંડ આપવામાં નહિ આવ

- Fees Structure: (Includes Daily Food Coupon to Men & Women)
- Fees should be submitted along with this Form which is Non –Refundable in any case.
- Last date for Form Submission is 31/03/2026 up to 6.00 pm.
- Entry Fees will be paid in IMPS / NEFT Only.
Intention of this tournament is to bring member closer & share happiness, kindly cooperate accordingly.
Misbehavior will not be tolerated at any cost and all decision will be made by JPC cricket committee only.
Decision power for cancellation of the tournament due to any unforeseen circumstances will be with management only.
